Job Summary
Our Family Medicine practice provides comprehensive primary care, including physical examinations, routine health screenings, and preventive care for patients of all ages, while also supporting providers whose practice is focused exclusively on adult patients. Our goal is to identify and manage health risk factors before they become more serious concerns. We diagnose and treat both acute and chronic illnesses and promote overall health, disease prevention, and well-being.To enhance practice operations, increase patient access, and deliver an exceptional patient experience, this role will include a combination of traditional primary care visits and same-day sick appointments. Providers practice autonomously at the top of their licensure while collaborating closely as part of a supportive, team-based care model.
We are seeking an experienced, full-time 40-hour Nurse Practitioner or Physician Assistant to support our office located at 30 Tuscan Boulevard in Salem, NH. We are open to our patients Monday-Friday from 8:00 AM to 5:00 PM with flexible start and end times. The position includes 35 hours of direct patient care and 5 hours of administrative time each week. Providers are expected to build and maintain a patient panel. Once a panel is established, the schedule will transition to 32 hours of direct patient care and 8 hours of administrative time per week.
Summary
The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations. May also perform additional duties, such as precepting a small group of learners.

Essential Functions
-Provides direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
-Performs complete histories and physical examinations.
-Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
-Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
-Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
-Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
-Performs bedside procedures as are appropriate to the patient population.

Master's Degree Nursing required

Experience
