Home Health / Hospice Aide

Join Saint Agnes Home Health and Hospice, part of Trinity Health At Home, and make a meaningful impact by providing compassionate care to clients in their homes.

Responsibilities:

• Assist with personal care (bathing, grooming, dressing, mobility)
• Light housekeeping, meal prep, and laundry
• Monitor vital signs and report changes
• Support medication self-administration and equipment safety
• Maintain accurate documentation and communicate with RN supervisor

Qualifications:

• High school diploma/GED
• Must be a Certified Home Health Aide (HHA) in the state of California
• Federally compliant training and competency evaluation
• 1+ year experience in home care, hospice, or institutional setting
• Driver’s license and reliable transportation
• Ability to lift up to 35 lbs
