This position is eligible for relocation depending on location.

Clinical pharmacistsare responsible foroptimizing pharmacotherapy regimens, ensuring safe patient care practices, andadvocating forevidence-based medicine within interdisciplinary healthcare teams.As decentralized team members they will conduct daily medication therapy reviews, consults, and other clinical services for patients within designatedarea(s) of responsibility or targeted populations served such as med-surg, telemetry, oncology, pediatrics, critical care, or emergency medicine.They will follow established policies, procedures, protocols, standards, and regulatory requirements in executing job responsibilities as well as contribute to the pharmacy’s mission to improve health outcomes in our community.

Clinical pharmacists will be assignedarea(s)of coverage based on experience, training, and competency.Rotate day and eveningshifts;every third weekend.

  • Appropriately assesses patient data toidentifymedication-related problems through profile reviews, pharmacyconsultsand interdisciplinary rounding.

  • Participates in medical emergencies and other formal alerts such as trauma, stroke,sepsisand STEMI asrequired.

  • Reviews and interpretsprovidersorders to assure that the drug, dosage form, and dose isappropriate forthe patient's age, weight, and comorbidities. Checks for drug incompatibilities andallergies, andtakesappropriate actionwhen necessary.

  • As necessary, consult with providers, nurses and other healthcare professionals involved in patient careregardingpharmacotherapy optimization with intent to achieveoptimalpatient outcomes across the continuum of care.

  • Providesaccurateandtimelyresponses to other healthcare professionals and patientsregardingdrug information questions and facility medication use policies toassuresafe andappropriate useof medications.

  • Serves aspreceptorfor pharmacy students/residents andassistswith orientation, instruction and training of pharmacy students, interns, pharmacy technicians, residents,pharmacistsand other health professionals.

  • Assistsnursing with medication administration (oral, injectable, continuous infusion titration) depending on the work assignments andin accordance withregulatory and hospital policies.

  • Documentspatient care activities including therapy plan,monitoringand follow-up in electronic medical record as per departmental guidelines.

  • Responsible for the completion of all assigned work during the shift, whilemaintainingthe highest standards of quality and safety through the completion of quality control checks,identifyingand reporting out medication errors and near misses,identifyingprocess improvements and ensuringappropriate documentation.

  • Maintains current awareness and adheres to State and Federal laws, regulations, pharmacy policies, standard operating procedures,formularyand pertinent clinical practice guidelines while providing pharmaceutical care to patients.

Schedule: Full Time Rotating Shift

General Requirements:

  • Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.

  • Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residencyor3 years of hospital experiencerequired.

  • Successful completion of PGY-2 residencyor3 years of experience in the relevant clinical practice area preferred.

  • BLS certification within 3 months of hire.

  • ACLS and/or PALS certification within 3 months of hire based on practice area(s).Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire.

  • BPS Board Certification (BCPS or specialty board certification in the relevant clinical practice area) within 2 years of hireor10 years of experience in the relevant clinical practice area.

  • Other certifications if requested by direct supervisors/managers.

  • Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.
