MercyOne Medical Group – Northeast Iowa is made up of more than 30 primary care, pediatric, internal medicine and specialty clinics located throughout Black Hawk, Bremer, Buchanan, Benton, Butler, Fayette and Tama counties.
Under the supervision of a Registered Nurse or Licensed Practical Nurse, works collaboratively with the health care team to provide outcome/goal-based patient care. Observes andreportsthe patient’s response and progress to interventions. Adheres toMercyOne'sperformance expectations and performs all defined services and other related dutiesin accordance withthe Mission,Visionand Values ofMercyOne.
ESSENTIAL FUNCTIONS
Supports the team inidentifyingpatient needs and implementing the outcome/goal-based plan of care.
Knows expected outcomes/goals for each patient andassiststeamto achieve.
Assistsin monitoring patientcondition – (i.e.,temp, vitals, blood glucose monitoring, etc.)
Assistsin educating patients and significant others (i.e., hygiene, positioning, ambulating, etc.) under the direction of the RN.
Assiststhe patient care team in the delivery of care.
Sets up rooms to receive patients.
Assistsin admitting patients and orienting them to the hospital routine.
Performs allappropriate functionsof the PCA role when needed to function as a “sitter” for a patient.
Makes patient call lights on unit a priority and responds promptly.
Provides bathing, oral hygiene, toileting needs andassistsin feeding.
Assistswith prevention of skin breakdown by taking the initiative to reposition patients regularly and ambulate as directed.
Transports patients safely.
Demonstrates competency in unit specific tasks and seeksassistancewhen necessary.
Responds to telemetry calls promptly toassureongoing monitoring capability.
Participates in"hourlyround" process toassurepatient safety and that patient needs are being met.
Adjusts to changes in team/patient priorities asdeterminedby the team leader.
Assistsothers, outsideimmediateteam, whenownwork assignmentcompleted.
Communicates relevant clinical information to others tooptimizepatient outcomes/goals.
Activelyparticipatesin team communication tomodifythe outcome/goal-based plan of care and to keep theCardexinformation current.
Reports observations of patient(s) responses to care, (i.e., tolerance of ambulation, etc.)
Reports and documentsaccuratepatient information such as vital signs, weights, I&O, blood glucose, specimens, calorie counts, etc.
Immediatelycommunicatesimportant informationto the team leader/RN such as changes in condition, abnormal vital signs, questions about care, familyquestionsor concerns, etc.
Communicates with other departments when needed (i.e.telemetry, dietary, etc.)
Documents completely,accuratelyand legibly and in accordancetopolicy.
Performs unit-based functions as assigned.
Completes shift duties as assigned (i.e., cleaning lists, temperature checks, refrigerator temperatures, etc.).
Cleans equipment indirtyutility room before end of shift and returns to storage area.
Makes rounds in all assigned rooms toassuredirty linen removed, items stocked as needed and room tidy.
Removes equipment/supplies from rooms upon discharge.
Collects and applies age-specific data.
Performs tasks/procedures, uses equipment in anage appropriatemanner.
Communicates and works with others in anage appropriatemanner.
Adheres to all safety, infection control and colleague health policies and procedures.
Attends required safety programs.
Appropriately and consistentlyusesthe personal protective equipment requiredofthe job.
Knows and candemonstratedepartmental emergency procedures for fire,disasterand other emergency situations.
Activelyparticipatesin regularly scheduled drills (Code Red, Green, etc.) and regulatory safety standards.
Completes and appropriately routes incident reportsin a timely manner.
Usesequipment safely and appropriately.
Reports and removes unsafe equipment/furniture.
Asks forassistancewhen needed toassuresafety for all involved.
Maintains a safe working environment and practices safe working habits.
Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and proceduresin order toensure adherence in a manner that reflects honest, ethical, and professional behavior.

Minimum Qualifications
Ability to read, write, speak, and understand English.
Effective communication with patients, visitors, staff, and physicians.
One of the following required:
Completion of Nurse Aide I course (verified by transcript or certification)
Completion of Nursing Fundamentals course
Completion of Trinity Nursing Assistant Program (TNAP)
Successful completion of Competency Test
Iowa EMT certification
(Nurse Aide II – Advanced preferred; may be provided post‑hire. Home Health requires Nurse Aide I.)

Non‑Violent Crisis Intervention (NVCI) within 6 months of hire (Mental Health/CD areas)
