Our home care caregivers provide professional and empathetic support across Northeast Ohio, divided into separate teams to better provide coverage. This role will provide this support to the Greater Cleveland Area – Cleveland’s West, Downtown and East locations as far as Vermillion. As an RN on this team, you willdeliverthe best possiblecare to patients in the comfort of their own homes.In this role, you will provideassessments, complextreatmentsand education to help avoid readmissions to the acute care settingand ensure comfort.This position offers a flexible schedule, allowing you tomaintaina healthy work-life balance, all while changing the lives of your patients and their loved ones every day.
A caregiver in this position is working PRN and would need to be flexible from 12:00am - 12:00pm.
A caregiver who excels in this role will:
Provide direct nursing carein accordance withestablished healthcare organization policies,procedures,and protocols.
Implement andmonitorpatient care plans.
Provide supervision to LPNs,HomeCareAidesand families/caregivers involved in the care of the patient.
Performaccurateand complete admission assessments and reassessments within a specifiedtime frame.
Assesspatientfor safety risks,abuseand neglect.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Assess and document patients' pain usingappropriatepain scale and manage patients'pain then.
Prioritize and document patient problems based on assessed needs and problems in collaboration with patients,familyand healthcare team.
Maintain complete and up-to-date documentation and files at all timesregardingpatient condition and effectiveness and timeliness of care and interventions.
Escalate change in patient condition to patient’s physician asappropriate.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school ofProfessionalNursing
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS)Certification through the American Heart Association (AHA)or the American Red Cross
One year of clinical experience as an RN in an acute medical/surgical setting
Minimum one year current clinical experience as a registered nurse
Minimum one year experience in an acute medical-surgical or home care setting
ValidDriver’sLicense(VDL) in Ohioand proof of automobile insurance with $100,000/$300,000 coverage
Preferred qualifications for the ideal future caregiver include:
Bachelor’sofScience inNursing (BSN)

Physical Requirements:
- Requires full range of body motion including handling and lifting packages, manual and finger dexterity, and eye-hand coordination.
- Requires standing and walking for extensive periods of time.
- Heavy work – Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment as required.
Pay Range
Minimum hourly: $41.00
Maximum hourly: $41.00
